Physical Therapist (PT) – Full Time – South Bend & New Carlisle, IN 

Up to $15,000 Sign-On Bonus

We are seeking a full-time Physical Therapist to join our skilled and patient-centered teams at senior living communities in South Bend and in New Carlisle, IN. This is more than just a job—it’s a chance to help residents thrive through personalized therapy, while working in a supportive, therapist-owned organization that truly values clinical excellence.

• Flexible full-time schedule (Monday–Friday, 30–40 hours/week)
• Compensated drive time and mileage for travel between the two locations
• Strong  interdisciplinary support and welcoming therapy team

About the Facilities:

South Bend:  Located on south side of town, this senior living community offers a full continuum of care and a vibrant, team-focused culture.

• Continuing care retirement setting: short-term rehab, long-term care, outpatient, and assisted living.
• Well-equipped therapy gym has ADL kitchen and bathroom.

New Carlisle: Centrally located in the community, the beautifully landscaped campus creates a friendly, home-like environment.

• Continued care rehab community, with multiple levels of care, assisted living, and skilled nursing facility.
• Three therapy gyms: for PT, for OT, and an independent workout room. Training kitchen and bathroom.

What You Bring:

• Active Indiana Physical Therapy license (or eligible to obtain)
• Graduate of a CAPTE-accredited Physical Therapy program
• Passion for working with older adults and improving quality of life
• Strong communication and collaboration skills

Requirements

Applicants must have an active Indiana Physical Therapist license or must have the necessary qualifications to earn Indiana license. These qualifications include earning an advanced degree from a Commission on Accreditation in Physical Therapy Education-accredited physical therapist education program, passing the state licensure exam, and fulfilling the state’s requirements for licensure.

Applicants must be legally eligible to work in the United States. Proof of eligibility will be required upon offer of employment.
