About the role

Description

Position Summary:

The Health and Wellness Coordinator supports members in the Behavioral Health System by promoting active participation in their health through programs like MIFitKID. This role focuses on delivering targeted wellness activities, coordinating care with families and providers, and fostering long-term well-being through personalized, community-based support.


Essential Duties/Responsibilities:

· Support members by actively engaging in their own health and wellness through the implementation of MIFitKID Programming.

· Plan and deliver targeted interventions and activities that address individual needs, leverage strengths, and support meaningful outcomes.

· Collaborate with care teams and other support professionals to ensure comprehensive, coordinated services for enrolled members.

· Empower members by offering education, guidance, training, and connections to community resources and natural support systems.

· Partner closely with children and their families to ensure a collaborative, family-centered approach to care.

· Focus on achieving functional, measurable outcomes that enhance daily living and overall well-being.

· Honor and integrate the unique cultural values and heritage of each child and family into all aspects of care and support.

· Organize and lead recurring site and community-based events that promote health, wellness, and community engagement.

· Manage and maintain an active caseload, ensuring consistent, personalized support for each member.

· May be required to work scheduled evening and weekend hours to meet the needs of families.


Requirements

  

Education and Experience:

· High School Diploma or G.E.D. is required. 

· Training certification or health/exercise related bachelor’s degree preferred.


Required Skills/Abilities: 

· Must have a passion for Health and helping others.

· Must be able to work independently in the community.

· Help others improve their lives through preventative and healthy lifestyle design.

· Working knowledge of basic typing/computer skills. 

· Working knowledge of Microsoft products.

· Effective written and verbal communication skills.

· Excellent organizational skills.

· Extensive knowledge of behavioral health agency policies and procedures preferred.

· Ability to maintain personal and professional limitations.

· Ability to understand the decision-making process and completion of forms related to confidentiality.


Travel required: 

· Providers will work from a range of work locations in their community. Locations may change throughout the course of employment and travel up to 50% of the time.

· Ability to operate vehicle during the course of a workday and transport members.

· Provide services to parents/caregivers in the home and/or community.


Physical Requirements and Work Environment: 

· Prolonged periods of sitting at a desk and working on a computer.

· Must be able to lift up to 50 pounds. 

· Constantly stooping, bending, standing and walking during a shift.

· Must be able to be work in both indoor/outdoor working conditions including extreme weather (heat/cold). 

· Must be able to remain in a stationary position 15% of the time.

· Occasionally moves about inside the office to access filing cabinets, office machinery, etc.

· Occasionally operates a computer and other office machinery, such as copy machine and printer.

· Occasionally ascends/descends stairs to visit other offices or attend meetings.

· Occasionally positions self to reach desk drawers, file cabinets, etc.

· Frequently communicates with internal and external constituents related to compliance-related issues, policy interpretation, compliance training, etc.


Additional eligibility requirements:

· Current Fingerprint Clearance or the ability to obtain a clearance card. 

· Must have a valid and current Arizona Driver's License.

· Must have reliable transportation. 

· Must meet vehicle insurance requirement (50/100 liability) and vehicle registration. Must have a clean driving record.

· This position requires work authorization in the United States.
