Position Summary:
(Overall purpose of this position)
- Organizes and manages the needs of patients and their families associated with Behavioral Health referrals. Provides linkages for the benefit of the patient and family.

- Screen and process mental health referrals in EPIC, including making outreach phone calls and sending letters. Manage all referrals through completion/closure. Link referred patients to BH providers and other services outside of NFP as indicated.
- Prioritize and manage EPIC in-basket requests to the BH department, including prescription refill requests and messages.
- Prioritize and manage BH department phone calls, including calls directed to the Psychiatrist and Clinical Nurse Specialist.
- Maintain current list of community resources, updated periodically, available both for clients and staff.
- Provide interpretation services (Spanish) for the psychiatric team and to other NFP providers as needed.
- Quickly fill any openings in the provider schedules, with priority given to psychiatry appointments.
- Assist the other mental health staff in treatment of our clients, both individually and in group. This will also include case discussion, collaboration and planning.
- Make reminder phone calls to patients, with priority to psychiatry appointments.
- Order transportation as needed to decrease barriers for patients.
- Work closely with front office staff to manage BH call volume and to transition scheduling to the front.
- Triage and manage requests from Providers, Nurses, Medical Assistants, Patient Advocates, PBS and Billing regarding patients’ BH needs.

- Computer proficiency: Microsoft Office Suite, Web maintenance, database management.
- LSW or comparable degree preferred.
- Familiarity with community resources.
- Fluency in Spanish and English.

- Ability to see, communicate, hear and utilize electronic communication devices.
- Office work environment.
- Demonstrated ability to walk, sit or stand for long periods of time. Requires some lifting papers or boxes up to 50 pounds.
- Work environment is stressful at times.
- Required to work one evening per week.
Previous experience that is necessary background to qualify for this position:
- Experience working in a behavioral health setting preferred.
