About the role

Essential Functions

  • Track daily client schedules of follow-up care.
  • Keep record of client activity – including refusals, no shows, rescheduling, arrived on time, etc.
  • Follow up to ensure completion in required time frame of admission – including, but not limited to, completion of H&P’s, Psych Evaluation completion. Any documentation not complete in required time frame will be notified to Director of Nursing, or designee.
  • Schedule appointments for clients, assign each provider daily appointment schedule.
  • Works closely with Behavioral Health Associate department to ensure clients are present for their appointments and other needs.
  • Follows up on level of care changes, ensures appropriate LOC paperwork in client chart.
  • Schedule clients on provider’s schedule for new requests, pop-up meetings, peer to peer reviews or treatment teams.
  • Ensures that client charts, MAR and vitals are gathered prior to provider seeing client.
  • Contact MD offices to gather medical records of clients.
  • Cross train in office coordinator duties, including, but not limited to: calling local medical, x-ray, dental, vision offices for consults, following up regarding insurance questions.

Requirements

  • HS Diploma/GED Required
  • First Aid & CPR Required
  • Experience working in healthcare required
