Description
The Urgent Care Practitioner will see patients presenting without a scheduled appointment who are not easily accommodated within the clinical service areas to ensure that patient’s medical needs are met at the time of the visit, either by providing the necessary clinical services as an independent practitioner, referring the patient to her/his primary care practitioner as appropriate or otherwise working with the other members of the clinical staff to assist patients in navigating the range of services available on-site to achieve a positive clinical outcome and a high-quality experience for all patients seen.
RESPONSIBILITIES AND DUTIES:
- In collaboration with other members of the clinical staff, provide unscheduled patients with immediate attention to presenting complaint(s) in a timely and clinically appropriate manner.
- Provide health education and counseling regarding a variety of topics including the nature of the presenting complaint(s), appropriate follow-up, available on-site referral services, and the importance of scheduling and keeping regular appointments.
- In collaboration with the Patient Services staff, ensure that patients presenting for unscheduled care receive on-site referral appointments as appropriate to facilitate effective clinical follow-up for the presenting concern(s); assist in making appointments and accessing other on-site services as appropriate.
- In collaboration with members of WEALTH and other staff, provide health education one-on-one and in small groups, spontaneously (e.g., in the patient care waiting areas) and on a scheduled basis (e.g., during health fairs).
- Provide coverage as needed in clinical units at BMS-Main and at other clinical sites, as needed.

Eligible if worked 21 or more hours per week. Some rules may apply.

Requirements
- Family Nurse Practitioner certification preferred.
- Experience and/or training in a community health center setting
- Strong interpersonal skills
- Strong problem-solving skills
- Bilingual (Spanish, French, Haitian Creole) a plus
- Computer literacy/proficiency required.
- Board Certified and or Board eligible.
- Minimum of 2 years' experience.
