We are seeking a Clinical Team Facilitator to join our dedicated home health team. In this vital role, you’ll support patient care delivery by coordinating services, managing communications, ensuring accurate documentation, and collaborating closely with clinical staff and physicians.

What You’ll Do

As the Clinical Team Facilitator, you will:

  • Coordinate home health services, labs, equipment, and supplies per physician orders and client needs.

  • Manage incoming and outgoing calls, ensuring timely, accurate communication with providers, staff, patients, and families.

  • Accurately document physician orders, care plan updates, and all coordination activities in the EMR.

  • Assist schedulers and care managers in arranging and adjusting patient visits based on changing needs.

  • Follow up promptly on lab results, medication changes, and physician communications.

  • Support process improvement initiatives to enhance patient safety, quality, and operational efficiency.

  • Triages operational and clinical calls appropriately and maintain calm, professional communication even in urgent situations.

  • Provide coordination during non-business hours as assigned.

  • Uphold organizational values, ethical standards, and continuous quality improvement practices.

Minimum Qualifications

  • Graduate of an accredited LPN/LVN program

  • Active LPN/LVN license in the state of practice

  • Two or more years of experience in home health, acute care, or ambulatory care

  • Experience using electronic medical records and office software

  • Valid driver's license and reliable transportation

  • Strong verbal and written communication skills

  • One to three years of customer service experience

  • (California only): Minimum one year of professional nursing experience required by regulation

Physical and Work Requirements

  • Frequent standing, lifting, and walking

  • Ability to perform detailed tasks with frequent interruptions

  • Occasional patient positioning and lifting

  • Adaptability to shifting priorities and potential on‑call hours
