Job Details
The primary function of the Diabetes Educator is to plan, manage and implement diabetes educational programs throughout the Network.This position requires providing service to the following age populations: Adult, Older Adult, Frail Elderly in a manner that demonstrates an understanding of the functional/developmental age of the individual served.
The coordinator of the program will have experience in managing chronic disease, facilitating behavior change as well as have experience with clinical and program management. The coordinator of the program will provide outpatient diabetes and self-management training and education to diabetic patients. The coordinator will plan, develop and implement diabetes education classes for adults, their families and caregivers. Instruct patients on the Standards of Care for diabetes and provide nutritional counseling for weight control and disease management. Instruct patients, family members and care givers on how to administer insulin, self-monitor glucose and medication education. Maintains the program accreditation a and all documentation required.The coordinator will develop collaborative relationships with other departments, services and community health care agencies to facilitate support and assist in development of policies and procedures for diabetic health education guidelines.

Job Description
Education/Training/Experience:
Three years of direct patient contact including patient diabetes education is preferred.
Licenses/Certifications:
Registered Nurse (Active PA License) or Registered Dietitian or health professional with a Bachelor’s degree who can be eligible to take the Certified Diabetes Educator exam is required.
Certified Diabetes Educator preferred.
