POSITION SUMMARY

The Mobile Assessor / Family Counselor is responsible for providing assessment services for those clients who are seeking outpatient treatment services and who find one of our IOP offices to be a convenient location. The Mobile Assessor will work closely with the referral sources in coordinating a seamless transition of care for clients needing further treatment. The Mobile Assessor / Family Counselor will also be responsible for performing assessments in a hospital or clinical setting as needed. This position will also present the various programs of Cumberland Heights to area schools, hospitals, physicians and counselors in private practice. The IOP Mobile Assessor is responsible for performing initial screening, assessment, pre-certification when indicated and completion of all paperwork for potential clients and serves as a liaison between the patient and the admissions staff. As family counselor, this employee provides psycho-education and structured group and individual counseling and/or therapy to patients and/or families impacted by substance use disorders. The Mobile Assessor / Family Counselor also covers groups in the absence of the primary counselor. This position coordinates care with IOP primary counselors and Directors and is part of the IOP treatment Team. 

                 

PRIMARY DUTIES AND RESPONSIBILITIES include some or all of the following:

  1. Screens inquiry calls and conducts face to face chemical addiction assessments to determine appropriateness for admission, within scope of practice as defined by licensure and clinical privileges.
  2. Provides provisional diagnoses based on assessment findings within scope of practice as defined by licensure and clinical privileges.
  3. Communicates clearly with each prospective patient their treatment options and, to the extent possible, their probable financial responsibility.
  4. Meets scheduled appointments in a timely and professional manner.
  5. Determines appropriate level of care for each person assessed for services within the scope of Cumberland Heights programs, or makes an appropriate documented referral for persons who would best be served in a different program. 
  6. Communicates and cooperates with admissions, benefits verification and UR personnel and provides timely and complete documentation of the admission to UR and data entry personnel.
  7. Coordinates clinical consultation with the appropriate Clinical Manager regarding the admission appropriateness of persons who may have co-occurring psychiatric disorders or medical instability.
  8. Documents clinical activities in patient record according to established policies and procedures.
  9. Ensures that potential patients and referral sources are given accurate and complete information about the services offered by Cumberland Heights, and when possible, introduces potential patient to the care provider with whom the patient will be working. 
  10. Facilitates individual, group and family counseling and/or therapy sessions, within scope of practice.  
  11. Assesses clinical problems and develops treatment plan, in conjunction with the patient.
  12. Participates in daily and weekly team meetings and treatment plan review.
  13. Provides educational components relative to addiction and recovery to patients and families.
  14. Communicates regularly with family members, Employee Assistance Professionals, physicians, referrals, judges, probation officers, etc. regarding treatment progress and continuing care needs.
  15. Recommends ways to improve the quality and delivery of services.
  16. Maintains confidentiality of company and patient information.
  17. Reacts productively to change
