Description
The Unit Coordinator assists with the coordination the discharge planning for SNF patients and arranges services needed upon discharge from the SNF.
The Unit coordinator supervises the HB/SNF activity program. Has full access to patient health information.
Shift: M-F, 8:00 AM - 5:00 PM
Requirements
High School diploma or equivalent.
Successful completion of a Certified Activity Director course.
Knowledge of Medicare, Medicaid, and SNF state/federal regulations preferred.
Certification in BCLS from American Heart Association or American Red Cross.
Must possess full range of body motion to pass basic FIT test for position to include walking, kneeling, standing, pushing, pulling, bending, stooping, reaching and sitting for extended periods of time.
Must be able to lift and carry up to 30 pounds.
