Clarity on the Role: 

This position serves as a member of the Wellvana Pharmacy Team. Responsible for abstracting and analyzing pharmacy reporting Health Plan data. Will work as in conjunction with the Clinical Pharmacist on ongoing process for Pharmacy Quality Initiatives. The Pharmacy Tech will conduct daily member outreach for Rx related assistance and education, as well as any additional duties deemed necessary for the Pharmacy Department. 

What's Expected: 

• Perform medication reconciliation in ambulatory locations  
• Contact retail, mail order, specialty pharmacies to ensure medication lists are accurate 
• Perform scribing functions for the providers 
• Retrieve Pharmacy reports from Health Plans portal for distribution to the practice 
• Ensure patients can pick up refills for chronic medications 
• Review medical claims to confirm medication delivery to patients. 
• Collaborate with Clinical Pharmacist to ensure optimal communication with Market Leads. 
• Keep accurate documentation of medication changes and patient progress. 
• Collects, summarizes and trends provider performance data to identify and strategize opportunities for provider improvement. 
• Collaborates with Provider to improve Quality Performance (claims and encounters). 
• Supports Provider office-based quality improvement by identifying missed members-provider follow ups, affecting medication refills. 
• Communicate with local and online Pharmacy to determine reason for missed or late Rx pick up. 
• Reach out to members with historical missed refills to determine obstacles. 
• Keep track of obstacles causing missed Rx refills to determine solutions and aid. 
• Daily member outreach for Upcoming refill reminders. 
• Daily member outreach for Past Due refills affecting Part D Scores. 
• Report weekly tasks performed, and productivity based on Market. 
• Performs other duties as assigned. 

Requirements

 Education (Degree/Major):  

• Bachelor’s degree or equivalent experience. HS Diploma Required. 

Certifications: 

• Certified Pharmacy Technician (CPhT). 
• Medical Scribe Certification 

Years of Related Experience:  

• 2 years retail experience or performing medication reconciliation at a hospital or ED. 
• 1 year of Medical scribing 

Skills: 

• Proficient in database and spreadsheet applications (Microsoft Excel, Words, etc.). 
• Excellent organizational, interpersonal, and communication skills. 
• Ability to handle sensitive or confidential information. 
• Knowledge of Medicare Part D Measures. 
• Willing to travel if needed.
