Under minimal supervision and at the direction of the Case Management Director, the Inpatient Social Worker/Patient Counselor is responsible for assisting patients in meeting their bio-psychosocial needs while enabling them to access and utilize medical care and community resources in order to achieve their optimal level of health. The Social Worker/Patient Counselor performs crisis intervention and short-term counseling for patients and families and transitions patients along the continuum of care. The Social Worker/Patient Counselor performs discharge planning for identified patients by coordinating internal and external resources in collaboration with the patient, family, and health care team.

Required Qualifications:
- Master's Degree in Social Work or Counseling Psychology.
- One (1) year experience as a Social Worker or Counselor.
Preferred Qualification:
- Two (2) years Hospital experience.
