Position Highlights:
- Position: Clinical Pharmacy Specialist Medication Reconciliation
- Location: Northwest Community Hospital – Arlington Heights, IL.
- Full Time: 40 hours/week
- Hours: Monday-Friday, Hours vary upon the need of the department
General Summary:
Reporting to the Clinical Pharmacy Manager with accountability to the Director of Pharmacy and Pharmacy Manager, the Clinical Specialist is responsible for participating in and executing all functions related to clinical pharmacy programs. Responsible for provide pharmaceutical care which provides safe and effective drug therapy to improve patient outcomes.
What you will need:
- Education: BS Pharm orPharm D degree.
- License: Current license to practice pharmacy in the State of Illinois.
- Certification: Board Certification required or in process within 24 months of start date.
- Experience: One year post graduate residency preferred or a minimum of three years experience in area of specialty in lieu of residency
- Unique or Preferred Skills: BLS and ACLS certification within 3 months of hire required.
