Physical Therapist

Job Description: 
We are seeking a compassionate and skilled Physical Therapist to join our team at our Skilled Nursing Facility in McMinnville, Oregon. This is an exciting opportunity to make a positive impact on the lives of our residents while working in a supportive and collaborative environment. The successful candidate will work directly with patients to develop and implement individualized treatment plans aimed at improving mobility, strength, and overall physical function. 

Key Responsibilities: 

• Perform evaluations and assessments of residents to develop personalized treatment plans. 

• Provide direct patient care, utilizing evidence-based interventions to improve physical function. 

• Educate patients and their families on exercises, pain management, and mobility strategies. 

• Document patient progress and communicate effectively with interdisciplinary teams. 

• Collaborate with nursing staff, physicians, and other healthcare professionals to provide comprehensive care. 

• Supervise and mentor physical therapy assistants and support staff. 

• Adhere to all regulations and standards of care within the Skilled Nursing Facility setting. 

Qualifications: 

• Current, valid Physical Therapist licensure in the state of Oregon. 

• Degree from an accredited Physical Therapy program. 

• Strong interpersonal and communication skills. 

• Ability to work independently and as part of a team. 

• Experience in a Skilled Nursing Facility or similar healthcare setting preferred but not required. 

• Compassionate and patient-focused approach to care.
