About the role

Summary: 

Responsible for providing administrative support to hospice clinical staff in a manner that ensures efficient, quality delivery of care for patients and families. 
Supports the hospice team with scheduling visits, staffing assignments, preparation for clinical and educational meetings, and processing documentation for the patient's medical record.

Monday thru Friday

7a - 3:30p or 7:30a - 4p (rotating)

Job Responsibilities: 

1) Coordination of paper and electronic documentation flow from the teams. Ensures integrity of the medical record and efficient flow of documentation. Assists with the collection of documentation for clinical audits

2) Supports payroll by updating timekeeping system for manager review and entering mileage and work times.

3) Schedules hospice visits with the Hospice Medical Director and other hospice providers.

4) Performs scheduling duties and assigns cases to appropriate IDT members. Tracks and reviews field staff schedules daily. Review daily visit information and notes for accuracy and timeliness. Demonstrates flexibility and creativity in responding to staffing needs created by emergencies, scheduling changes, and adverse conditions. 

5) Assures staff schedules are full to maintain productivity standards and works with the manager to ensure performance concerns are addressed promptly

6) Completes workflow tasks as assigned in the electronic medical record, including assigning daily visits, admissions, rescheduling, and processing missed visits.

7)  Supports the referral process and related staff when necessary

8) Completes scheduling in collaboration with Clinical Manager for after-hours staff and physicians

9) Demonstrates and maintains effective communication with all team members.

10) Copies/collates/distributes written communication to designated personnel as needed and requested

11) Maintains patient confidentiality at all times. Responds to patient, family, and physician telephone calls professionally and efficiently. Answers, records, and relays messages in a timely manner using appropriate etiquette.

12) Sets priorities in work, separates emergency scheduling from regular workflow, and completes assignments within the required timeframe with minimal supervision or need for follow-up.

13) Demonstrates proficiency with Microsoft Office and the organization's computer systems and computer-related activities. Enters clinical data into the computer system accurately and timely as appropriate. 

14) Develops, maintains, and distributes daily reports for communication of pertinent information to management and hospice team member

15) Provides administrative support for interdisciplinary team meetings, including but not limited to documentation retrieval, scanning, and processing, preparing reports, and follow-up with staff to ensure compliance and accountability

16) Supports providers with administrative tasks and processing of documentation

17) Scans and uploads paper documentation timely and accurately into patient's medical records, and maintain a system to track and obtain any outstanding documentation.

18) Ensures contracted facilities receive patient documentation timely, accurately, and within regulatory standards.

19) Assists Patient Care Coordinators in smooth transitions of care throughout multiple care settings

20) Other duties as assigned

Required Work Experience: 

3 years of hospice, home health, or other medical experience

Preferred Work Experience: 

Excellent customer service, both oral and written and organizational skills. Utilization of electronic medical record systems. Staff scheduling
