Wage range is $23.00 - $25.50/hour based on experience.
What You'll Do:
Vista Prairie at North Pointe is seeking a Community Care Coordinator to join our compassionate team. As a dedicated Community Care Coordinator, you will embrace a resident-centered approach, treating every individual with unwavering dignity, regardless of their disease, diagnosis, or progression. While providing a therapeutic environment that maintains each resident’s highest level of physical, social, and physiological well-being, you will find ways to bring smiles to the faces of those you serve. You will also answer resident emergency calls promptly, assist in transporting residents to/from appointments, activities, meals, and social programs as necessary. As a Community Care Coordinator, you will also assist residents in preparing for activity and social programs as necessary and much more! Together, we can make a meaningful impact on the lives of our residents.
Schedule:
- Day Shift (6:00 AM to 2:00 PM)
What You'll Need:
- High school diploma or GED required.
- Training and/or experience in health-related disciplines preferred.
- 1 year of supervision experience preferred.
- Healthcare setting experience is beneficial as well as medication administration and direct care.
- Demonstrated ability to read, write, and carry out directions required.
