Licensed Social Worker (LSW)- HomeCare

We are seeking a compassionate and experienced Licensed Social Worker (LSW) to join our Home Care team! This role is responsible for supporting patients and families through assessments, care coordination, advocacy, education, and community resource management to promote overall well-being and quality of life. The ideal candidate is organized, detail-oriented, and passionate about providing exceptional patient-centered support in a collaborative and caring environment.
Minimum Requirements:
• Current Florida Licensed Social Worker (LSW) credentials required
• Knowledge of laws, regulations, and guidelines related to Senior Living and Home Health
• Minimum one year of experience in a healthcare setting preferred
Knowledge, Skills & Abilities:
• Strong organizational and communication skills
• Detail-oriented with the ability to maintain accuracy daily
• Excellent interpersonal skills with a professional and compassionate approach
• Proficient in Microsoft Office programs including Excel, Outlook, and Word
• Ability to learn and use proprietary software as needed
