The Cadence Health team seeks Proactive Titration Clinical Navigator, as part of our expanding team, to play a critical role in enhancing the patient experience and ensuring the smooth delivery of care. The Proactive Titration Clinical Navigator will be responsible for contacting patients to discuss disease specific medication recommendations in collaboration with Cadence NPs and the patient's Physician.

This role involves contacting patients during core business hours: Full-time, 10:00 AM - 7:00 PM or 11:00 AM - 8:00 PM Eastern Time.

WHAT YOU’LL DO:

  • Reach out to patients to review medication titration recommendations provided by Cadence Nurse Practitioners, ensuring clear communication and adherence to treatment plans.
  • Every healthcare interaction with Cadence must be an exceptional experience that prioritizes patient well-being and aligns with our health system partners' goals.
  • Take ownership of your tasks while ensuring high standards of accuracy and professionalism.
  • Maintain precise, detailed documentation of patient interactions in compliance with HIPAA and Cadence's clinical protocols.
  • Use your expertise to guide patients in understanding disease-specific medications and how they relate to their care plans.
  • Show up on time and consistently meet deliverables, ensuring patient care and service are never compromised.
  • Provide timely, clear, and empathetic communication with patients and healthcare providers.
  • Manage your responsibilities independently while maintaining high performance and efficiency.

WHAT YOU’LL NEED:

  • Active LPN license in a Compact Nursing State, with eligibility to practice across multiple states under the Nurse Licensure Compact (NLC)
  • 5+ years of clinical experience in acute or ambulatory care, with a strong preference for experience in cardiology or nephrology settings.
  • Experience maintaining HIPAA-compliant environments and protecting patient confidentiality.
  • Must have a HIPAA-compliant home office setup with reliable internet, including stable wifi and strong upload/download speeds.
  • Highly skilled in written, verbal, and interpersonal communication, with the ability to engage patients and healthcare providers effectively.
  • Strong track record of working independently, managing time effectively, and fulfilling job duties with minimal supervision.
  • Consistent attendance and adherence to work schedules, ensuring timely and effective patient care.
