Job Summary:
Cardea Health is search of an organized and motivated person to work as a nurse practitioner as part of a mobile Substance Use Disorder treatment program targeting individuals experiencing homelessness. The NP will work in an interdisciplinary team focused on providing harm reduction and treatment options to people experiencing homelessness residing in shelters, transitional housing, and other residential settings.
The NP will engage with individuals with substance use disorder around SUD treatment options, including education, prescribing, monitoring and follow up. The NP is expected to collaborate with the interdisciplinary team and the participants medical team in the management of medication assisted treatment. This includes an assessment and diagnosis of a client’s behavioral and medical health needs and recommendations for ongoing treatment and care.

Qualifications & Responsibilities
Responsibilities: The nurse practitioner will assess program participants for eligibility for medication assisted treatment of substance use disorder and prescribe accordingly. Duties include:
· Direct supervision of Substance Use Navigators (2) and Harm Reduction interventions conducted across program sites
· Maintains strong working knowledge of DSM5 diagnostic criteria, specifically for substance use and severe mental illness (SMI)
· Evaluate interested program participants for eligibility for medical treatment of substance use disorder. Provide appropriate medical treatment of substance use disorder until care is successful transferred to a community provider
· Assure that connections are made to services and supports based on individual choice and clinical assessment
· Develops and maintains collaborative relationships with providers, facilities, community resources, etc. and facilitates warm hand-offs as needed
· Maintains accurate and up to date documentation as required by program deliverables and departmental, local, state and federal policy requirements
· Participates in clinical and administrative supervision, case conferences, staff meetings, in-service training, and continuing education
· Practices harm reduction principles, including interventions that minimize harmful effects of substance use while treating all clients with dignity and respect
· Participate in data collection as required by program administration
· Other duties and responsibilities as required
Required Qualifications:
· Bachelor's Degree in Nursing Science and an advanced degree in a healthcare science as required by the State Licensing program
· Minimum of two (2) years of full-time experience, or its part-time equivalent, working as a nurse practitioner
· Completion of an accredited Nurse Practitioner program
· American Nurses Credentialing Center (ANCC) Board Certified as a Nurse Practitioner
· Licensure as an Advanced Practice Nur
