About the role

At the present time, a vacancy exists for a Case Manager - Supervisory within the Student Services division of the Chester County Intermediate Unit. This is a project position, working with Home and Community Services, responsible for arranging services to clients of Home and Community Services (commonly referred to as "Wraparound services") for pre-school and school aged children diagnosed with emotional/behavioral disorders. The incumbent will guide and assist eligible individuals in gaining access to necessary medical, educational, rehabilitative, and social services; monitor service delivery; and evaluate outcome and continued need for services. The Home & Community Case Manager will be responsible for: developing a client-centered plan; promoting positive relations with referral, regulatory and funding agencies; and promoting the integration of children with disabilities in the community. The Home & Community Case Manager will participate in the intake process, selection, orientation, clinical supervision and training of clinical staff, including Therapeutic Staff Support, Behavior Specialist Consultants, Mobile Therapists and Case Managers. This position may include some direct service to clients such as Mobile Therapy and Behavioral Specialist Consultant at the discretion of the program supervisor. This position is located at the Educational Services Center in Downingtown, PA.

Summary of Position:

The Case Manager - Supervisory will directly report to the Behavior Supports Coordinator - IBHS Program. The Case Manager - Supervisory will oversee all staff (MT, BC-ABA, BCBA, BC, BHT-ABA, BHT) assigned on their caseloads and provide clinical supervision to BHT-ABA workers (minimally 2 groups) and BC-ABA,BA/MT staff as assigned. Because of the nature and location of the services provided, the incumbent will have a high level of "freedom to act" which will require a person who is self-energized, self-reliant and able to make decisions within the arena. The incumbent will need to apply technical, consultative and implementation skills in identifying, assessing, coordinating and implementing services and supports for the child/family.

Duties and Responsibilities:

  1. Oversee the development and implementation process of individual treatment plans as well as monitoring and revising the plans.
  2. Conduct service intakes for clients.
  3. Work with a consultant pool for the purpose of evaluating clients to determine their medical need and eligibility for Home & Community (Wraparound) services.
  4. Meet with the Interagency Team, including the child/family, to develop a summary of care.
  5. Coordinate and oversee delivery of services delineated in plan of care summary whereby individualized plans can be carried out.
  6. Monitor the billing system to ensure reimbursement from Medicaid (both through the Chester County managed care organization and fee-for-service) and private insurers for services provided to clients.
  7. Maintain a strong professional relationship with the professional community.
  8. Assist Coordinator in maintaining contact and dialogue with early intervention, school district, and other community organization personnel to assure that there is a working relationship, which will enable the child/adolescent to function optimally in these settings.

Qualifications: 

  • Masters degree in an area related to Education, Social Work or Mental Health. required.
  • Board-Certified Behavioral Analyst Certification required
  • Licensed mental health professional; or
  • An individual with a Master's degree and at least one-years experience in a CASSP system (employed by or under contract to Children and Youth Services, Juvenile Justice, Mental Health, Special Education, or Drug and Alcohol working with children) school, or employed by a licensed mental health services agency or subcontracted agency.
  • Ability to communicate clearly and concisely
  • Ability to transport between central office and various locations throughout the county and state as required by the position.
  • Must have an understanding of the mental health and Medicaid systems.
