Clinical Assessor RN - (PRN) - Must Reside in Southern Counties of New Hampshire

Position Overview

Join us as a Nurse Assessor, where your role will be pivotal in educating and assessing program participants across vibrant communities in New Hampshire. You'll conduct medical eligibility assessments and nursing facility evaluations, engaging with clients in Nashua and throughout the southern counties. Your focus will involve performing field assessments predominantly in Nashua, New Hampshire, making a tangible impact on the lives of those we serve.

This PRN role requires a dedication of 20 to 30 hours weekly, presenting a distinctive hybrid structure that integrates remote work from your home office and fieldwork. Notably, the majority of your time will be spent working within our communities and in direct interaction with patients in Nashua and neighboring counties, fostering a dynamic equilibrium between remote responsibilities and hands-on involvement in the field.

Job Responsibilities:

• Review clinical documents and perform meticulous medical eligibility assessments, both within community and nursing facility settings. Evaluate and determine precise levels of care required by patients, considering their individual needs.
• Identify essential community-based resources and services required to address diverse health needs. Analyze psychological, emotional, behavioral, and physical aspects to formulate precise care recommendations.
• Ensure adherence to program and company policies in all daily operations. Cultivate good professional relationships with colleagues, management and ensuring the delivery of patient-focused care at its finest.
• Attend scheduled trainings and meetings, maintaining an updated knowledge base for clinical reviews and assessments.
• Foster effective collaboration among healthcare professionals, patients, families, and support networks to ensure holistic care of patients.
• Efficiently prioritize and execute tasks with acute attention to detail, ensuring optimal outcomes and timely delivery of services.
• Embrace a dynamic role by accommodating additional duties as needed, as directed by management, to enhance service delivery.

Required Qualifications and Experience:

• Hold an active unrestricted Registered Nurse (RN) License within New Hampshire, alongside an Associate’s or Bachelor’s Degree.
• Minimum of 2 to 3 years' experience conducting patient clinical assessments
• Demonstrate a strong sense of responsibility, accountability, self-motivation, and meticulous attention to detail.
• Possess robust medical and clinical assessment skills, coupled with exceptional verbal and written communication abilities.
• Exhibit independent judgment to address and resolve complex problems hindering service provision. Proactively identify and resolve obstacles.
• Work effectively in a team environment, contributing towards accurate, timely, and quality-driven assessments.
• Showcase proficiency in PC/Computers, Microsoft Office, internal software applications, databases, multitasking across multiple screens, and adept data entry skills.
