Social Worker - Licensed Clinical Therapist

Nemours is seeking a Social Worker to join our Nemours Children's Health team in Jacksonville, FL.

 

The Social Worker is necessary to deliver comprehensive patient and family-centered health care.  The position will support the medical care team in addressing the social determinants of health in Pulmonology. Moreover, the incumbent in this position will respond to all manner of crises in the clinic including positive suicide screens, suspected abuse and neglect, impaired parents/caregivers, bereaved families, food insecurity, and transportation disadvantaged. Many patients and families present to clinics with complicated psychosocial needs pertaining to the social determinants of health.  The consequences of unmet attention to psychosocial considerations in the health care setting are far reaching and include diminished patient satisfaction, higher no-show rates, decreased levels of treatment adherence, health care transition gaps, poor school performance, and untreated mental health problems.  Moreover, as Nemours moves to systematic screening for social determinants of health and suicidality, it becomes essential to have appropriately trained and available staff to respond to positive screenings and concerns that will likely be surfaced.

 

Essential Functions

1. Additional miscellaneous duties and responsibilities, as may be assigned from time to time by employee’s supervisor.
2. Conducts a psychosocial assessment for each individual/family assigned. The assessment should identify patient and family strengths, coping styles and problem areas within the context of their diagnostic, social, financial, cultural and psychological situation.
3. Assists families in integrating the medical, social and emotional dynamics of the disease/treatment into the context of their current life situation.
4. Documents social work interactions in the EMR in a timely manner. 
5. Coordination and linkage with community resources and agencies relative to health, crisis intervention, disability, schools, etc. 
6. Provides support for patients and their families assisting with emotional, personal, psychological, and socioeconomic factors along with adjustment to illness issues. This includes the entire spectrum from diagnosis, throughout the treatment phase, as well as the bereavement process, if necessary. 
7. Provides ongoing coordination of services to assist children with long term chronic illnesses in managing the impact of the illness through the various phases of childhood, adolescence and/or transition into adulthood.
8. Responds to the needs of culturally diverse and socially, intellectually or physically challenged patients.
9. Provides consultation for physicians and other members of the multidisciplinary healthcare team to address psychosocial needs in the context of adjustment to the illness, treatment, and treatment adherence. 
10. Establishes and maintains appropriate interpersonal boundaries with patient and family.

Requirements

Master's Degree

Licensed Clinical Social Worker (LCSW) credential

One to three years of experience required; Previous experience in hospital or healthcare setting preferred.
