PURPOSE:

To provide routine daily nursing care and services that support the care delivered to patients/residents requiring critical access acute care, in accordance with the established nursing care procedures and as directed by the supervisor.

ENVIROMENT: 

Lincoln Community Hospital and Nursing Home is a 15-bed Critical Access Hospital co-located with 22-bed Long Term Care Unit. The facility employs 130-200 employees. 

ESSENTIAL DUTIES AND RESPONISILITIES:

1) Provides care as directed by the professional nurse to patients/residents requiring long-term, rehabilitative care or restorative care.

2) Reports observations and other pertinent information related to the care of the patient/resident.

3) Documents objective information related to patient/resident care.

4) Assists with registration of patients/residents in the computer.

5) Provides services that support the care delivered to the patient/resident.

6) Provides input in the formulation and evaluation of standards of care.

7) Participates in the development of unit objectives.

8) Participates on the identification of staff educational needs.

9) Conducts self in a professional manner in compliance with unity and facility policy.

10) Participates in the quality assessment and improvement process and activities.

11) Performs other related duties as assigned or requested.

EXPOSURE RISK:

The Certified Nursing Assistant (CNA) is at high risk for exposure to blood and body fluids or other potentially infectious materials.

SUPERVISION RECEIVCED:

Receives administrative supervision from the Director of Nurses, or the Assistant Director of Nursing. May receive functional supervision from registered or practical nurses working on the unit.

SUPERVISION EXERCISED:

Exercises functional supervision, responsibility, and accountability necessary for carrying out assigned duties. 

MINIMUM QUALIFICATION:

1) Graduation from high school.

2) Previous nursing assistant experience or student nurse preferred.

3) Completion of certification program for nursing assistants preferred.

a. Nursing Assistants can work for 4 months following the completion of a class before testing.

MINIMUM PERFORMANCE STANDARDS:

Performance in the following areas is acceptable when:

General Patient/Resident Care:

1) Patients/residents are identified by the use of the identification band prior to administering treatments, serving meals, etc., as necessary. 

2) Patient/resident is approached in a kind, gentle and friendly manner. Respect for the patient’s/resident’s dignity and privacy is consistently provided. Confidentiality of all patient/resident information is maintained. 

3) All grievances and complaints made by the patient/resident are reported to the Charge/Staff Nurse.

4) Assistance is provided with admissions/transfers/discharges.

a. Room is stripped after patient/resident is transferred/discharged.

b. Patients/residents are moved to accommodate unit needs.

5) Specimens are transported to the appropriate areas

6) Patient/resident rooms are re-stocked.

a. Rooms are neat and orderly.

b. Excess supplies and linen are removed. 

7) Care plans are reviewed daily to determine if changes in the patient’s/resident’s daily care routine has been made.

8) Assistance with patient/resident care is provided and directed.

a. Baths are given.

b. Linens are changed.

c. Patients/residents are properly positioned.

d. AM and PM care is given.

e. Vital signs are taken.

9) Assistance with restorative programs as indicated.

10)  Hallways are cleared of equipment or on one side.

11) The unit is maintained in an orderly fashion.

12) Meal trays are distributed and picked up.

13) Patient/residents are assisted or fed at meal times as directed.

14) Bed pans, urinals and emesis basins are distributed and removed as requested.

15) Routine specimens are collected as directed.

16) Assistance with treatments is provided as directed.
