Registered Nurse - Huntington's Disease Support Services (503099a)

The Team:

As the Registered Nurse dedicated to providing specialist care for Tasmanians with Huntington’s disease (HD) you will have the opportunity to grow and support the Tasmanian Government’s HD service in North West Tasmania, supported by a highly skilled medical workforce including the TAZREACH program.

HD is a hereditary autosomal-dominant neurodegenerative disease characterised by motor, cognitive and psychiatric symptoms and at present, there is no cure for HD or treatment to slow the underlying disease process. Symptomatic treatment targets specific features that can be improved to reduce suffering and increase quality of life. Supportive care is also essential.

HD should be considered a life-long condition that has a profound impact not only on the consumer but also their families in many domains. This includes caregiver burden, economic hardship, transgenerational trauma, issues associated with living at risk of the condition, and family planning.

The Role:

The Registered Nurse – Older Persons Mental Health (Huntington’s Disease):

• provides a specialist service within the community setting for a designated client group, utilising best practice principles, person centred, and trauma informed care, within a collaborative and multidisciplinary framework.
• as part of a multi-disciplinary team will assess, support and monitor the health status which includes comprehensive biopsychosocial, active therapeutic engagement, monitoring and review of mental state and response to treatments of people with the Huntingtons Disease gene, or with Huntington’s Disease, and their carers, working constructively in collaboration with other  health care professionals and community sector organisations within an identified region via an assertive case management approach.
• strengthens health outcomes through the provision of safe quality, clinically appropriate nursing care in partnership with patients/clients, their families and other health professionals and community sector organisations within the defined community practice area. 
• Facilitates and the access of those with Huntington's Disease and their families to relevant services including the National Disability Insurance Scheme (NDIS), Centrelink, allied health, primary care, and specialist appointments, and relevant Community Sector Organisations (CSOs). This may include making applications to the NDIS or Centrelink.
• Educate health professionals and care providers and other services about Huntington’s Disease.

You will need 

1. Demonstrated ability to develop, implement and evaluate comprehensive care plans within a multi-disciplinary team environment.
2. Ability to undertake client education in the practice setting, together with a commitment to participate in ongoing professional development.
3. Demonstrated written oral and interpersonal communication skills, including the ability to work effectively in a multi-disciplinary team environment, communicate effectively with other sectors of mental health services and primary health care providers.

Details of appointment:

Permanent full time, day worker position, working 76 hours per fortnight, commencing as soon as possible.

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $78,179 to $105,995 per annum. Our Employer 11.5% superannuation contribution is on top of this amount.

• Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

Access to the Grade 4 salary range $99,455- $105,995 is subject to successful application for progression to Grade 4

In addition, the following allowances calculated on the salaried incremental point may be available:

• Post Graduate Allowance of up to 10 % based on qualification type
