Position Description:
 

Responsible for the admission assessment of the patient admitted to the hospital through the admitting office. This assessment will focus on Part I of the Patient Admit Assessment identifying relevant medical history and current medical problems, as well as, psychologic, social, environmental, self-care, educational and discharge needs necessary for initiating a plan of care that will meet the individual patient needs. Concurrently facilitates proactive patient care services to ensure that care is appropriate. Identifies, monitors and reports opportunities for quality and performance improvement to the appropriate department. The case manager takes an active role in performance activities as it relates to their area of assignment.

Qualifications:
Possess effective verbal and written communication skills.
Must be licensed to practice as a registered nurse in Louisiana.
