Home Health Director of Clinical Services

We are looking for a experienced Home Health RN Clinical Director to join our team. Must hold an active Arizona Registered Nurse license. The right candidate should be self-motivated, driven, out-going and efficient with their charting and review of documents.

Monday-Friday 8am-5pm mostly in office but some of the job will require patient visits and routinely keeping up with supervisory visits with our active field staff.

Requirements

At least 2+ years of Medicare Home Health experience required.

Excellent understanding of Home Care regulations, OASIS and PDGM.

Manage clinical Staff and direct reports.

Ensure compliance with all state / federal regulations.

Assist Administrator as needed.

• Case Opening Assessments
• Care plans
• Medication set up and reconciliation
• Wound care visits
• Supervisory Visits and patient visits
• Timely paperwork completion that meets DOH Compliance Standards
• Must be willing to travel within our main coverage area
• Positive team player who will also advocate for our clients and their needs - Make a Difference!

• Current AZ State RN License, TB test, CPR
• Clear Criminal Background Check and Drug Screen
• Detail oriented, multi-task, organized with good time management skills
• Reliable transportation, AZ Driver's License & Auto Insurance
• Experience with Wounds, Assessments, Medication set-up & Management
• Current and reliable cell phone and be easily reachable
