The Admissions Patient Coordinator serves as a key patient-facing role within the Admissions Department, supporting patients throughout the Initial Home Health Care admissions and authorization process. This position is responsible for coordinating physician appointments, nursing assessments, home visits, caregiver onboarding, and patient communication to ensure timely and effective initiation of services.


The Admissions Patient Coordinator provides education, support, advocacy, and ongoing communication to patients and families throughout the admissions process. Responsibilities include attending physician appointments, coordinating scheduling and documentation, collaborating with interdisciplinary teams, supporting onboarding activities, and ensuring patients receive a positive experience from initial approval through care initiation.


This role requires strong organization, communication, problem-solving, and time management skills, as well as the ability to work effectively in a fast-paced healthcare environment while maintaining a patient-centered approach to care. Candidates should be compassionate, adaptable, detail-oriented, and comfortable collaborating with patients, caregivers, healthcare providers, and interdisciplinary teams.

Responsibilities:

  • Serve as the primary point of contact for patients throughout the Initial Home Health Care admissions and onboarding process, ensuring consistent communication, support, and advocacy.
  • Manage an independent caseload of patients through the admissions, authorization, and onboarding process while maintaining timely workflow progression and follow-up.
  • Coordinate physician appointments, nursing assessments, home visits, caregiver onboarding, and required documentation to support timely initiation of home health care services.
  • Conduct home visits to establish patient relationships, provide education regarding the home health program, assess patient needs, gather documentation, and support onboarding activities.
  • Attend physician appointments as a patient advocate, facilitating communication between providers, patients, and internal departments while obtaining required signatures, documentation, and physician orders.
  • Collaborate with patients, families, physician offices, Resource Nurses, Authorization Specialists, Writing, Staffing, and operational departments to support continuity of care and timely workflow progression.
  • Obtain, upload, route, and track required documentation, including visit notes, Plans of Care (POCs), Letters of Medical Necessity (LMNs), assessments, and physician documentation.
  • Coordinate Durable Medical Equipment (DME) requests, communicate patient needs to appropriate departments, and educate patients on equipment expectations and usage.
  • Provide courtesy, milestone, and follow-up calls regarding approvals, onboarding status, scheduling updates, authorization progression, and care initiation.
  • Maintain accurate documentation, workflow updates, provider information, and patient records within Salesforce, EMR systems, and internal company platforms within required timelines.
  • Assist with caregiver onboarding, including communication regarding credentialing requirements, scheduling, onboarding tasks, and readiness for patient care assignments.
  • Identify barriers, workflow delays, patient concerns, or compliance issues and escalate concerns appropriately to leadership or supporting departments.
  • Collaborate closely with Admissions Leads and interdisciplinary teams while independently prioritizing tasks and managing competing responsibilities in a fast-paced environment.
