No Two Days Are the Same!
One day you may be coordinating caregiver schedules, helping a family arrange services, and conducting resident follow-ups. The next, you may be training a new caregiver, accompanying a resident to an appointment, helping with errands, or stepping in to ensure a resident receives the support they need.

Highlights of the Care Coordinator| Lead Home Health Aide position:
• Coordinate caregiver schedules and resident service requests
• Ensure residents receive exceptional care and personalized support
• Mentor, train, and support Home Health Aides and caregivers
• Assist with staffing, scheduling, documentation, billing support, and service coordination
• Conduct resident satisfaction follow-ups and quality assurance visits
• Help maintain continuity of care and service excellence
• Provide direct resident support when needed, including:
- Personal care assistance
- Medication reminders
- Transportation and escort services
- Grocery shopping and errands
- Light housekeeping and laundry
- Companion care and social engagement
- Pet care and dog walking
