$2,000 sign on bonus for eligible hires
Schedule:
Full-Time: 40 hours/week (1.0 FTE)
Monday - Friday, day and evening shift rotation (shifts between 5:00am - 9:30pm)
Every 4th weekend rotation

Job Summary:
Interview patients to collect medication histories and assist with the medication reconciliation process. Assists with the medication reconciliation process in coordination with pharmacy, nursing, care management, and other healthcare professionals as appropriate.Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
- Completes medication histories for new admissions. Assesses patient, caregiver or other identified persons to determine the most appropriate and accurate source of medication history and interviews. Follows up with pharmacies, nursing homes, caregivers or others as necessary to obtain complete medication history. Consults a pharmacist as necessary.
- Promotes the hospital’s retail pharmacy program through the offering of discharge prescription services if applicable. May coordinate the filling and delivery of prescriptions and discharge counseling as appropriate.
- Investigates discrepancies in the medication histories and other problems and follows up with pharmacists to facilitate the resolution of any issue.
- Documents, collects and compiles data, statistics and other information as required for quality assurance programs and assists with reports to provide information to facilitate leadership decision-making.
- Trains new users on the use of the Pyxis Automated Dispensing cabinet and CII Safe.
- Works in a constant state of alertness and safe manner.
- Performs other duties as assigned.
EDUCATION
- High School diploma/GED or 10 years of work experience
EXPERIENCE
- Two years experience
PHYSICAL REQUIREMENTS
- Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
- Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
- Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
- Frequent keyboard use/data entry.
- Occasional bending, stooping, kneeling, squatting, twisting and gripping.
- Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
- Rare climbing.
