About the role

We are seeking an experienced nurse educator to join our Advanced Heart Failure Comprehensive Care Center team. In this role, you will play a vital part in educating and empowering patients and staff, ensuring they have the knowledge and skills to navigate advanced heart failure therapies and post-transplant care. Your expertise will be instrumental in streamlining the discharge process and maintaining regulatory compliance.
  • Educate patients on advanced heart failure therapies, including VAD, TAH, and heart transplant, ensuring they understand their health condition.
  • Provide timely and comprehensive education to post-transplant patients and post durable MCS implant patients.
  • Streamline the discharge process for post-transplant and durable MCS patients, ensuring an efficient and smooth transition.
  • Educate hospital staff, including nurses, physical therapists, and physicians, on AHFT and MCS devices like VAD, Impella, TAH, and ECMO.
  • Create and implement educational programs for all ancillary staff, ensuring they are well-equipped to support patients.
  • Collaborate with partner units' leadership to maintain regulatory compliance, especially regarding TJC educational requirements.
  • Stay updated on the latest advancements in heart failure therapies and post-transplant care to provide accurate and relevant education.
  • Develop and maintain positive relationships with patients, fostering a supportive and educational environment.
  • Assist in the development of educational materials and resources for patients and staff, ensuring accessibility and clarity.

Provide education to patients and families as part of the Advanced Heart Failure Therapy evaluation process, particularly regarding VAD, TAH and heart transplant.  Support Preimplant and transplant Listing process.

Works closely with the Advanced Heart Failure Service Providers and Multidisciplinary team to educate patients and families post transplant and durable MCS devices as to their health conditions and the management of the transplant or the durable device to facilitate an efficient and optimal discharge process.   Support clinical teams to optimize and coordinate discharges.

The Nurse Educator will work closely with partner unit leadership and Nursing Connect educators to provide the necessary knowledge and hands-on practice to be able to provide safe care to MCS patients in the hospital including VAD, Impella, TAH and ECMO.

Identifies opportunities for improvement with the care of AHF CCC patients. Participates in quality improvement initiatives. Participates in protocol and policy development to promote best practices in heart transplant care  and MCS patient care.

Required qualifications:

  • At least four (4) years of Cardiac ICU nursing employment experience with evidence of strong understanding of cardiac hemodynamics, heart transplant and MCS devices.

  • Ability to design effective curriculum for a highly varied audience with different level of health knowledge. 

  • Ability to interact effectively with multidisciplinary teams and various leadership teams across the healthcare system. 

  • Progressive leadership or education experience as demonstrated by participation in unit councils, unit Performance/quality improvement initiatives or unit based education initiatives.

  • Demonstrated exemplary patient teaching skills and transferring of critical knowledge to patient and families including teach back and debrief.

  • Excellent organizational skills, proficiency in meeting deadlines, ability to work independently, prioritize workload and manage numerous concurrent tasks in high stress environment.

     

  • Demonstrated excellent written and verbal communication skills 

  • A compassionate and patient centered approach to education and care

  • The flexibility to orient and work at all UCSF Medical Center locations in person on campus

 

Preferred qualifications:

 

  • Master’s degree in Nursing 

  • 2 years Nurse Educator experience in a hospital system

  • 2 years expe
