Start of Care Registered Nurse (RN)

Millennium Home Care, a leading provider of compassionate home healthcare services, is seeking a skilled and dedicated Registered Nurse (RN) to join our team as a Start of Care Nurse. In this pivotal role, you will be responsible for initiating home health care services, conducting comprehensive OASIS assessments, and developing individualized care plans for our patients.

Responsibilities:

• Conduct thorough initial assessments and OASIS evaluations to determine patient needs and eligibility for home health services.
• Develop and implement comprehensive care plans based on assessment findings, collaborating with physicians and other healthcare providers.
• Provide skilled nursing care, including medication administration, wound care, and monitoring vital signs.
• Educate patients and caregivers about disease management, medication administration, and self-care techniques.
• Accurately document patient assessments, interventions, and outcomes in the electronic medical record.
• Collaborate with the interdisciplinary team to ensure optimal patient care and outcomes.

Qualifications:

• Active and unrestricted Registered Nurse (RN) license in the state of Florida.
• Minimum of 1 year of nursing experience, with a strong preference for home health or acute care.
• Demonstrated proficiency in OASIS assessments and documentation.
• Excellent clinical assessment, problem-solving, and decision-making skills.
• Strong interpersonal and communication skills to build rapport with patients and caregivers.
• Ability to work independently and manage a varied caseload.
• Proficient in electronic medical record (EMR) systems.
• Valid driver's license and reliable transportation.
