SCOPE OF RESPONSIBILITIES AND TASKS
General

Essential Duties and Responsibilities
1. The Pharmacy Director is expected to maintain, implement and practice the following, in addition to his or her administrative responsibilities.
2. AHF Ambulatory Care Pharmacist Practitioner’s Essential Functions
In general, AHF Pharmacists providing primary care to individual patients perform the following functions in collaboration with physicians and other members of the primary care team:
· Perform patient assessment for medication-related factors.
· Interpret data related to medication safety and effectiveness.
· Perform medication reconciliation of patients’ therapy.
· Provide information, education, and counseling to patients about medication-related care.
· Document the care provided in patients’ records.
· Identify any barriers to patient treatment and compliance.
· Participate in multidisciplinary reviews of patients’ progress.
· Communicate with payers to resolve issues that may impede access to medication therapies.
· Communicate relevant issues to physicians and other team members.
3. Medication Therapy and Primary Care Collaborative Practice Agreements (where appropriate).
Order laboratory tests necessary for monitoring outcomes of medication therapy.
Initiate or modify medication therapy care plans on the basis of patient responses.
Expanded Functions
1. Expanded primary care functions of pharmacist include all the functions previously described as well as:
Provide individualized health promotion and disease prevention, including administration of immunizations, diabetes and smoking cessations programs, as well as other Subject Matter Expertise (SME) programs as developed by AHF, with appropriate collaborative drug therapy management authority.
Includes prescriptive authority where this is legally and organizationally authorized such as PrEP and PrEP therapies.
