About the role

ESSENTIAL FUNCTIONS 

  • Provides regional (site-specific) non-patient care leadership of APPs.  Serves as a resource to APPs in addressing employee relations issues that may arise. 
  • Serves as a resource in the development of HOPCo Value-Based Care relationships. 
  • Works collaboratively with other Lead APPs and VP of Value-Based Care to effectively recruit and onboard new APPs 
  • Provides analysis of quality metrics. 
  • Conducts performance evaluations for APPs 
  • Assists in creating work schedules for APPs. 
  • Provides APP training and education 
  • Assesses patient health status including state of wellness, compliance with the care plan and determines appropriate diagnostic and therapeutic procedures. 
  • Provides acute and chronic patient care including taking histories, conducting physicals, monitoring therapies, and medications. 
  • Triages patient calls and evaluates patient problems. 
  • Triages emergencies  
  • Documents patient information and care provided inpatient records and maintains patient statistics. 
  • Educate the patients and families as appropriate. Provides continuity of care. 
  • Consults with other staff as necessary. 
  • Maintains patient confidentiality. 
  • The job holder must demonstrate current competencies for the job position. 
  • Other duties as assigned. 

EDUCATION 

  • BA/BS, MS preferred. Graduate of AMA accredited physician assistant or nurse practitioner program. 

EXPERIENCE 

  • Experience in orthopedics is required.   

REQUIREMENTS 

  • Current state APP license, certified by National Commission on Certification of Physician Assistants or Accreditation Board of Specialty Nursing Certification (ABSNC), current CPR certification.   
  • At least one year of experience in an APP leadership role. 

KNOWLEDGE  

  • Knowledge of acute and chronic protocols, administration of medications, delivering physical care following the treatment plan. 
  • Knowledge of how to assess patient health and measure/record physiological and growth indices. 
  • Knowledge of health promotion principles and techniques and patient learning measurements. 
  • Knowledge of quality metrics and standards. 

SKILLS 

  • Skill in gathering and analyzing physiological, socioeconomic, and emotional patient data. 
  • Skill in accurately evaluating patient problems in person or via phone and providing appropriate advice, intervention, or referral 
  • Skill in evaluating and revising care plans based on patient changes. 

ABILITIES 

  • Ability to make responsible decisions within the scope of the dependent medical practitioner. 
  • Ability to develop health education programs and materials. 
  • Ability to work collaboratively with all members of the health care team.
