About This Role:
The Basic 245D Service Coordinator is responsible for meeting with, setting up, and monitoring services for participant-directed Basic 245D licensed services offered, by Accra, through the State of Minnesota. The Service Coordinator provides customer service to participants which includes, but is not limited to, completing intake meetings, ongoing monitoring of services, explaining Direct Support Staff (DSP) employment and timecard processes, determining Responsible Party and DSP competency, and maintaining communication with the client, their designated representative(s), and county case manager. The Service Coordinator serves as a resource to other Accra employees about Basic 245D Services. The Service Coordinator is expected to stay abreast of current policies and changes to rules and regulations related to Basic 245D services, and to participate in committees and meetings to enhance their knowledge.
Requirements
Must meet one of the following requirements:
- Four- year degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.
- Two-year degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.
- Diploma in a field related to human services from an accredited postsecondary institution and three years of full- time work experience providing direct care services to persons with disabilities or persons age 65 and older.
- Minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications listed above.
Additional requirements:
- Demonstrated knowledge of services needed by people with disabilities and the elderly in Minnesota.
- Demonstrated creativity, critical thinking, and problem-solving skills.
- Able to affect change and demonstrate positive leadership.
- Knowledge of federal and state statutes applicable to services for people with disabilities and the elderly in Minnesota.
