This job monitors and evaluates drug therapy for appropriateness based on the patient’s health status, institutional criteria/guidelines, and cost efficient measures; assumes responsibility for the management of pharmacy services to assigned areas for the goal of improving patient outcomes; provides drug information to health care personnel; participates with appropriate medical team on patient care rounds; precepts pharmacy students and residents; and provides in-services/education to pharmacy and other health care providers when appropriate.

Education

Required – Graduate of an accredited School/College of Pharmacy, PGY1 Residency in Pharmacy Practice

Preferred – PG21 Residency in Pharmacy Practice

Certifications

Required – Licensed to practice pharmacy in the State of Louisiana, or eligible for licensure by examination or reciprocity from another state

Knowledge Skills and Abilities (KSAs)

• Must have computer skills and dexterity required for data entry and retrieval of information.
• Effective verbal and written communication skills and the ability to present information clearly and professionally.
• Must be proficient with Windows-style applications, various software packages specific to role and keyboard.
• Strong interpersonal skills.
• Ability to exhibit good clinical experience in specialty as well as leadership/teaching skills.
• Ability to understand the distributive process of the Pharmacy within 45 days from hire.
• Ability to travel to offsite locations as required.
• Ability to work flexible schedule as needed such as nights, evenings, weekends, etc.

Job Duties

• Performs patient assessment and care.
• Conducts Pharmacoeconomics.
• Educates staff, students, patients, health care providers and other health care professionals.
• Mentors residents, students and new staff to meet departmental goals.
• Assumes responsibility for professional development and continued education.
• Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
• Other related duties as required.
