The Department of Urology at OHSU is committed to providing the highest quality, state-of-the-art care for adult and pediatric urology patients. Function/Duties of Position
1. Clinical Activities (not related to assigned administrative, education, or research FTE teaching or research):a. Directi. Patient population: Inpatient: The APP will spend 60% of their time on our inpatient service and will be responsible for providing excellent direct clinical care for patients admitted to the adult urology service while working in conjunction with faculty physicians and resident physicians, during which time there will not be any competing outpatient responsibilities. Inpatient duties include but are not limited to: • Perform comprehensive histories and physical examinations and ongoing clinical assessments for admitted and post-procedural urology patients.• Evaluate, diagnose, and manage acute and chronic urologic conditions within the APP scope of practice in collaboration with the urology team.• Develop and implement diagnostic and treatment plans in collaboration with supervising physicians, consistent with credentialing and scope of practice.• Order, interpret, and act upon laboratory studies, imaging, and other diagnostic tests.• Admit, round on, and discharge patients, ensuring continuity of care throughout the hospitalization.• Write daily progress notes, admission notes, discharge summaries, orders, and other documentation in the electronic medical record in a timely and accurate manner.• Coordinate patient care with consulting services, procedural staff, and ancillary teams as needed.• Work collaboratively with nursing staff, pharmacists, case managers, social workers, therapists, and other members of the interdisciplinary care team to ensure safe, effective, and efficient patient care.• Participate in multidisciplinary rounds and contribute to discharge planning to facilitate timely and appropriate transitions of care.• Perform medication reconciliation and provide discharge instructions and patient education regarding diagnoses, procedures, medications, postoperative care, and follow-up plans.• Communicate patient status and care plans with patients, families, faculty physicians, resident physicians, consulting providers, and outpatient providers to ensure continuity of care.• Identify changes in patient condition requiring escalation of care and coordinate appropriate interventions.• Perform procedures within credentialed privileges, as applicable.• Complete accurate, timely, and compliant clinical documentation and billing, as applicable.• Participate in quality improvement, patient safety, and departmental initiatives as appropriate.
Outpatient: The APP will spend the other 40% of their time establishing and providing independent clinical coverage for the Urology Immediate Care Clinic, during which time there will not be any competing inpatient responsibilities. Duties include but are not limited to; • Evaluate, diagnose, and manage acute urologic conditions within the APP scope of practice to established patients within the department, including, but not limited to urinary tract infections, acute urinary retention, difficulty voiding, or post-operative concerns.• Order and interpret laboratory testing, urinalysis, urine cultures, imaging, and other diagnostic studies. • Develop treatment plans, prescribe medications, and provide procedural care within credentialed privileges. • Identify patients requiring escalation of care, emergency department evaluation, hospital admission, or physician consultation. • Communicate assessment findings, treatment plans, and follow-up recommendations with the patient's primary urologist and other members of the care team to ensure continuity of care. • Provide patient education regarding diagnosis, treatment, medications, self-care, and return precautions. • Complete accurate and timely documentation in the electronic medical record.• Collaborate with clinic nursing staff to address urgent patient concerns, provide clinical guidance, and provide orders as appropriate.ii.
