Hiring Range: $24.94 – $30.55 Per Hour

Kitsap Mental Health Services (KMHS) is seeking a compassionate and dedicated Client Health Advocate to join our Population Health team. As a Client Health Advocate, you will work closely with clients to support their health and wellness goals by offering coaching, empowerment, and guidance. You’ll help clients navigate accessibility barriers to health care services at KMHS and in the community, collaborating with medical providers and community resources. Primary Responsibilities:
- Advocate for clients to ensure they receive the necessary care and support, both within KMHS and through external resources.
- Assist clients in navigating the healthcare system, liaising with primary care providers and healthcare professionals, and tracking external referrals as needed.
- Promote health literacy by providing educational opportunities to clients and staff, ensuring clients can make informed health-related decisions.
- Collaborate with the Population Health team to design, implement, and promote health initiatives, including risk prevention, sex education, nutritional wellness, and more.
- Track and interpret data on health initiatives, conduct appropriate follow-up, and communicate with clients' care teams.
- Attend medical appointments with clients or facilitate client-centered conversations with medical providers to address barriers to care.
- Provide education and support to uninsured clients or those seeking changes in their health insurance coverage.
- Center health equity in all aspects of the role, assisting clients in addressing inequitable practices as needed.
- Maintain comprehensive and timely documentation of client progress for other providers' access.
- Participate in treatment team meetings to review client engagement and ensure re-engagement when necessary.
- Promote Diversity, Equity, Inclusion, and Accessibility in all client interactions and relationships.
Minimum Qualifications:
EDUCATION and EXPERIENCE:
Bachelor’s Degree in Social Services, Healthcare, Psychology or other related field accompanied by 2+ years of experience OR
High School Diploma or GED Equivalent accompanied by 4+ years of client-facing experience in the medical field, social services, or similar, OR
Certified Peer Specialist, Certified Medical Assistant, or similar certification requiring intensive training, if accompanied by 2+ years of experience.
This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must successfully complete a motor vehicle history check, possess and maintain a current, valid driver's license in the state of Washington, and have reliable, insured transportation.
LICENSURE: Agency Affiliated Counselor Registration (can be obtained after hire)
Preferred Qualifications:
EXPERIENCE: Experienced (minimum 3 years of job-related experience)
3+ years of experience working with underserved populations in the medical field, social services, or similar
Experience conducting one-on-one case management, creating and facilitating groups, and tracking client outcomes,
Experience documenting in and navigating Electronic Health Record (EHR) systems.
