Pay Scale:
AP: $19/hr.
QP: $23.08-$24.04/hr

Summary

Under indirect supervision, reviews and/or administers mental health and substance use screenings.    Provides comprehensive case management services to incarcerated persons.  Collaborates with community providers to facilitate referral and linkages to necessary resources upon release.  Conducts clinical evaluation/assessments including identifying diagnosis.  Provides crisis intervention services in a high-intense environment, which requires a high degree of independent decision-making. Communicates with law enforcement and other professionals to ensure the success of the program.   Monitors and follows up as required. 

Essential Duties and Responsibilities:

• Conducts and/or reviews screenings within 48 hours to determine mental health and/or substance use needs.  Provides comprehensive case management services for all individuals who meet program criteria.  Creates and updates transition/discharge plans based on individual needs and goals prior to release from jail. Collaborates with community providers to ensure discharge planning and concrete appointments address the following needs: mental health, substance use treatment, housing, food, medical care vocational/employment, transportation and legal assistance.  Conducts face-to-face assessments as clinically necessary and consults with law enforcement regarding program participants. 
• Communication with referred agency(s) to confirm appointment is successfully attended.  Communicate and coordinate care with Daymark Recovery Services Inc. Mobile Engagement Team when clinically necessary. Conducts post release surveys with program participants.  Tracks participation and reports on program outcomes.  Completes other documentation as required. 
• Initiates interdisciplinary treatment/transition planning and participates in treatment team meetings.  Engages in case support activities within Daymark as well as with law enforcement personnel and other community providers as deemed relevant to the position.  Creates and maintains community resource guide. 
• Other duties as assigned by the supervisor
• Provides assistance to the outpatient center as assigned by the supervisor.

The responsibilities of the Human Services Case Manager include, but are not limited to, the following:  

Mental Health and Substance Use Screening:

• Review and/or conduct mental health and substance use screening, document results
• Review information related to new arrests, communicate with law enforcement

Transition/Discharge planning:

• Identifies and documents the individual needs, strengths, resources, preferences, and goals to develop a transition plan for community integration.   
• Gathers information regarding all aspects of the recipient, including medical, physical, psychosocial, behavioral, financial, social, cultural, environmental, legal, and vocational/educational areas.
