About the role

The Director plans, directs and provides general supervision of SBH Health System Health Home Care Management Agency. They assume clinical supervisory functions including training, monitoring of records, evaluation of staff, and supervision of care managers. Responsibilities include compliance with all regulatory mandates, program growth, budget oversight and recruitment.

  1. Develops and implements policies and workflows for staff training, development and oversight.
  2. Develops and implements referral processes and integration workflows with other clinical services.
  3. Directs oversight of program resourcing and budget management.
  4. Develops and implements a program to audit program financials.
  5. Develops and implements a program to audit the quality of patient charts, and ensure regulatory compliance.
  6. Supervises administrative staff.
  7. Evaluates opportunities to enhance patient care and grow care coordination services, linking patients to outpatient care and social services, reducing hospital readmission and improving patient satisfaction and experience.
  8. Successfully engages with all levels of administrative and clinical leadership, clinicians, frontline and administrative staff.
  9. Monitors compliance with timelines, budgets, deliverables, and key performance indicators for assigned areas of responsibility.
  10. Evaluates processes and outcomes to identify system failures and develops corrective action plans.
  11. Performs other related duties at the discretion of senior management.
  12. Attends scheduled department and division meetings and participates as appropriate.
Qualifications
  1. Education Required – Master’s Degree in Social Work or Mental Health Counselling
  2. License(s) Required – LMSW, LCSW, or LMHC
  3. Experience/Skills Required –
  • Demonstrated 3-5 years of experience with case management and community-based programs providing services to HIV affected, dually diagnosed or mentally ill or substance use disorder populations.
  • Experience overseeing programs requiring compliance with regulatory standards and budget oversight including meeting productivity and case management targets.
  • Excellent collaboration, interpersonal and communications skills.
