What You’ll Do:

• Conduct thorough patient admissions, including assessments and care plan development.
• Provide education to patients and families on care plans, medications, and home health services.
• Collaborate with physicians and clinical teams to ensure smooth care transitions.
• Document all admission details accurately in the electronic medical record.
• Ensure compliance with regulations and company standards.

What We’re Looking For:

• Current California RN License.
• 1 Year Home Health Experience.
• Strong communication, organizational, and time management skills.
• Tech-savvy and comfortable using electronic medical records.
• Compassionate and committed to delivering exceptional patient care.

Schedule / Work Location: 

• Flexible weekends - Contra Costa County 

Experience:

• Home Health: 1 year (Preferred) 
• BLS Certification (Required)
• RN License (Required)
• Driver License (Required) 

Pay:

• Per Visit: $150-$230
• Hourly: $65-$82
