About the role

SUMMARY OF JOB DUTIES:
In collaboration with the Nurse Manager, Palliative and Complex Care and oversight of the Palliative and Complex Care Advanced Practice Provider and/or Primary Care Provider, the Palliative and Complex Care Coordinator expands the individualized primary healthcare services by evaluating certain acute or chronic health conditions for our Palliative Care organization.

ESSENTIAL JOB FUNCTIONS:
The Palliative Care Coordinator must be able to perform each essential duty satisfactorily. The listed items are representative of the knowledge, skill, and/or ability required:

  • Completes patient interviews, and accurately obtains and documents vital signs
  • Obtains laboratory specimen and gives injections per providers orders as directed using the appropriate techniques and demonstrates proper disposal of biohazardous waste
  • Handles portal messages from patients, refill medications, order additional labs or imaging as needed, complete prior authorizations, complete precertification for imaging, etc.
  • Maintains communication with Nurse Manager, Palliative Care, the Palliative Care Advanced Practice Provider, and the Primary Care Provider
  • Facilitates completion of telehealth appointments with the Palliative Care Advanced Practice Provider, as needed
  • Coordinates and maintains continuity with secondary services
  • Retrieves specialist records for Primary Care Provider review
  • Must have effective communication skills and instructs patient and/or their family as directed by the provider.
  • Call patients to triage, follow up, and discuss results as directed by the provider while maintaining patient confidentiality.
  • Travel to clinic locations and patient homes with the market
  • Retrieve and compile information
  • Research, analyze, interpret, organize, prioritize, and verify data and information
  • Maintain records/logs
  • Investigate, evaluate, and recommend action
Qualifications

MINIMUM REQUIREMENTS

  • Medical Assistant, Patient Care Technician, Health Coach or CAN exp required
  • 3 years of related home health or clinical experience and/or training is required; equivalent combination of education and experience considered as determined by the Nurse Manager, Palliative Care.
  • To perform this job successfully, an individual must have knowledge of Word Processing and internet software.
  • Athena EHR experience is preferred but not required.
