About the role

Job Summary

This is a full-time professional position. The Clinical Manager of Advanced Practice Providers provides clinical oversight and supervision for Advanced Practice clinicians at Transitions LifeCare. Manager is responsible for assuring effective daily operations, adequate staffing in all service areas, quality patient outcomes, market growth, and direct patient care. The role assists with creating and implementing care model delivery changes and collaborates with clinical and administrative leaders to ensure appropriate care for patients. This role assists in providing Palliative Care consultation, Hospice care and primary care to patients in hospitals, residential facilities, hospice inpatient facility and home settings. The schedule for this position is Monday-Friday 8:30am-5:00pm; on-call rotation; holidays as scheduled.

Administrative Responsibilities (30% of time)

  • Engage in and support organizational efforts to establish a culture that promotes the ideals of diversity, equity, and inclusion in all aspects of hiring, promotion, selection, and employee relations.
  • Provide clinical and administrative oversight and supervision to Transitions LifeCare APPs including, but no limited to, professional direction, hiring, orientation, coordination of training, staff development, performance management and appraisals, and schedule assignments (including on-call coverage)
  • Make recommendations and implement decisions on operational and resource issues that have department-wide impact
  • Assist with development, education, and implementation of technologies relevant to APP practice
  • Provide oversight to ensure compliance with regulations, standards, policies, and practices for APPs at TL
  • Participate in the interview process and orient all new NP/PAs
  • Participate in creation and continual review of applicable practice policies and procedures
  • Work closely with CMO to analyze and determine staffing levels and productivity in line with budget expectations
  • Identify new opportunities to expand services in collaboration with administration and key stakeholders
  • Collaborate with medical directors and CMO to establish clinical guidelines and processes
  • Participate in TL committees as needed
  • Assist with education/lunches/presentations
  • Complete minimum of 3 chart audits quarterly for each community-based NP/PA
  • Complete minimum of 2 ride-a-longs per year with each community-based NP/PA
  • Complete minimum of one day/quarter at WakeMed with WM NP/PAs
  • Complete initial and ongoing credentialing at WakeMed and Rex Hospitals

Clinical Responsibilities (70% of time)

  • Provide Palliative Care Consultation, Hospice visits and primary care to patients in residential facilities, home settings, Hospice Inpatient Facility and hospitals
  • Provide direct patient care on an ongoing basis as directed by the Chief Medical Officer, Hospice/Palliative Care Physician, referring physician, patient, family, and plan of care; utilizing pain and symptom management interventions that are based on established hospice and/or palliative care guidelines and standards
  • Provide attending MD status for Hospice patients when assigned
  • Provide Face to Face visits to ensure compliance with Hospice Medicare/Medicaid regulations when assigned
  • Attend IDG meetings as appropriate
  • Document in patient medical record to meet federal, state, regulatory, and agency guidelines
  • Work with the Chief Medical Officer and Hospice/Palliative Care Physicians to educate hospital staff, residential facility staff, and the community physicians about palliative care pain, and symptom management, communication around end-of-life decision making and other programs available through Transitions LifeCare
  • Participate in on-call rotation
  • Perform other duties as assigned by supervisor.

 

Requirements what you need:

  • Nurse Practitioner or Physician Assistant with current N.C. license
  • 3 years clinical experience required
  • 2 years experience in hospice, palliative care required
  • At least 2 years experience in supervision or leadership (does not have to be as an NP/PA at time)
  • ACHPN certification or testing within two years of hire required (for NP)
  • Demonstrated experience in the assessment of clinical services and needs of patient populations
  • Excellent verbal and written communication skills
  • Thorough knowledge of clinical operations in the field of hospice and palliative care, as well as familiarity with alternative payment models including value based and ACOs.
  • Maintain educational requirements to meet Nurse Practitioner or Physician Assistant standards.
