Position Summary

Under direction, this role works as a clinical expert clinician to provide heart failure services as part of the heart failure nursing team, providing a seamless service for patients, involving advanced clinical assessment, symptom management, medications management, patient education, and psychological care.

Job Responsibilities

  • Where applicable, establishes priorities for nursing care through patient assessment and develops nursing care plans. Prepares equipment and assists providers during examination, testing, and treatment of patients as needed.
  • Assists in the complex assessment and management of acute on chronic, complex heart disease including heart failure, arrhythmias, pulmonary hypertension, VADs and transplant patients; provides and coordinates comprehensive care and treatment.
  • Serves as the first line of triage in this chronically ill population.
  • Responsible for in-clinic management of ventricular assist devices (VAD) patients including driveline dressing changes in accordance with programmatic and JCAHO protocol.
  • Manages and interrogates VAD; identifies and escalates issues as needed.
  • Provides education to patients and families on pre- and post-VAD implants.
  • Responsible for in-clinic management of heart transplant patients. Utilizes specialized knowledge to recognize abnormalities in immunocompromised, complex patients to avoid adverse outcomes.
  • Closely monitors all specialized testing; being able to recognize abnormal/out-of-range lab values for immunosuppression.
  • Closely involved in JCHAO accreditation for all advanced heart failure programs; serves as expert point of contact during reaccreditation, audits, and site visits.
  • As needed, administers appropriate injections and medications as ordered by medical providers in compliance with state law and clinic guidelines.
  • Obtains prior authorizations and consents for specialty medications; educates patients on high-risk medications; provides routine monitoring of labs to determine each month’s eligibility for specialty medications
  • Conducts walk tests and performs calculations to assess aerobic capacity and endurance in evaluation of patients with various health conditions including pulmonary disease and heart failure.
  • Coordinates life-saving durable medical equipment (DME) which requires frequent dressing changes, batteries, and maintenance for device equipment, oxygen, etc.
  • Triages patient inquiries and communicates test results.
  • Ensures infection control compliance within the practice.
  • Participates in safety programs and orientation of new staff. Assists in reviewing and revising policies and procedures.
  • When needed, records medical information within the patients’ electronic medical record as appropriate.
  • Attends workshops, seminars and/or conferences to keep abreast of standards and best practices within the field. Disseminates information to colleagues and/or staff as appropriate.
  • If applicable, assesses patient eligibility and reviews patient medical history for inclusion in clinical studies.
  • If applicable, reviews and completes protocol history sheets, flow sheets, study follow-up reports and patient evaluation forms. Prepares and amends IRB protocols, consent forms and research documents.

Education

  • Bachelor's Degree in Nursing

Experience

  • Approximately 5 years of clinical nursing experience working with heart failure cardiology patient population.

  • Extensive experience as a Registered Nurse may be considered in lieu of the BSN.

  • Experience in chronic, complex heart disease including heart failure, arrhythmia, pulmonary hypertension, and ventricular assist devices (VADs).
  • Specialized knowledge in heart transplant to recognize abnormalities in immunocompromised, complex patients to avoid adverse outcomes.

  • Advanced clinical assessment and decision-making skills.
