Provides physical therapy treatment to patients who have impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease or other causes with minimal guidance from a Physical Therapist. Scheduling Options

• Monday through Friday, 8:00 AM to 4:30 PM

What You Will Do

• Deliver high-quality, evidence-based physical therapy treatments in patients’ homes

• Implement individualized treatment plans developed by supervising physical therapists

• Promote safety and functional independence in the home setting

• Educate patients and caregivers on exercises, mobility, and home safety

• Document patient progress in compliance with Vanderbilt Health and regulatory standards

• Coordinate care with nurses, physicians, and allied health professionals

• Travel to patient homes across designated service areas

Required Qualifications

• Excellent communication and interpersonal skills

• Ability to work independently and manage a flexible caseload

• Active Physical Therapist Assistant (PTA) license in Tennessee

• Minimum one year of experience as a Physical Therapist Assistant

• Valid driver’s license and reliable transportation

Preferred Qualifications

• Experience with electronic medical record systems

• Strong understanding of rehabilitation techniques and functional mobility

• Ability to work independently and manage a flexible caseload
