POSITION SUMMARY: To support the goals of BHcare by providing community support services which focus on developing individualized goals with consumers around a valued role that will increase their success and overall quality of life. This will be accomplished through skill building and motivational support for pursuing goals related to employment, education, community involvement and use of natural supports.
Accountable and responsible for the following results:
- Provide individual and/or group sessions while meeting caseload and productivity requirements.
- Conduct a functional assessment upon admission updated quarterly (WCMHN)or every six months (i.e. LIST).
- Develop a person-centered recovery plan with stage-wise treatment goals and objectives that are measurable and attainable and ensure it is reviewed at least every 3 months.
- Observe record and evaluate consumer’s progress towards goals.
- Document stage-wise interventions according to the psychiatric rehabilitation skill building model.
- Provide outreach & crisis intervention to community support service consumers as needed.
- Provide individual and group sessions in the community focused on the appropriate stage-wise intervention within the psychiatric rehabilitation skill building model developed by the agency (e.g. assisting with ADL’s, teaching recovery skills, develop self-advocacy skills and provide health and wellness education).
- Provide limited transportation as needed while clients learn individual transportation skills.
- Assist clients with connecting to medical and/or psychiatric appointments as needed.
- Provide education, support and consultation to family members, significant others and community members to meet the rehabilitation goal(s) set by the consumer.
- Develop individualized and generalized community supports.
- Consult with Program Manager whenever a consumer no longer requires the current level of care.
- Provide direct service in accordance with contractual guidelines.
- Adhere to the standards of care to ensure that the identified quality measures are obtained.
- Supply data in a timely fashion to the Director and Program Manager to fulfill reporting requirements.
- Required to use own vehicle on company business, or to transport clients.
- Attend training sessions, workshops, seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing.
- May be asked to provide case management, psycho-educational services, lead/facilitate family group, co-facilitate groups for IOP, orient new clients to the IOP, and participate in the development and ongoing process of an integrated service plan as program needs dictate.
- May be asked to administer breathalyzer/urine toxicology screens when necessary.
- Maintain clinical records in accordance with policies and procedures. Complete progress notes, treatment plan review, and discharges in a timely manner.
- Perform other duties as assigned
