ROLE AND RESPONSIBILITIES

The Children’s Focus Case Manager will assist families with linkages to program services,

treatment, and community support in an effort to prevent and manage crisis situations while

maintaining a focus on consumers achieving self-sufficiency despite their diagnosis and/or life

challenges. All work is performed in accordance with City, State and Federal regulations,

CATCH policies and procedures, and professional mental health standards.

BRIEF OUTLINE OF DUTIES

Provide short-term critical case management services to approximately 12 children and

adolescents (ages 4-21) who have been identified as meeting the Tier II criteria identified by

CBH.

The services provided will include, but shall not be limited to:

● Establish contact with the member/family within 24 hours of the intake date. Establish

face to face contact within 7 days and facilitate completion of the intake packet within 30

days.

● Conduct a meeting with the family monthly within 30 days, using data collection tools, to

obtain information regarding current needs and behaviors, functional strengths of the

family, identify supports and resources available to the family, and gain an understanding

of the family objective and vision in order to complete the Plan of Care.

● Conduct a family meeting every 30 days to review the Plan of Care goals, assess for the

current family satisfaction rating, and revise the plan of care to ensure that it reflects the

current needs of the family.

● Identify behavioral health challenges and psychosocial stressors, including social

determinants, that are problematic for the member or family. Partner with the family and

treatment team to create a crisis plan to address these challenges in a way that would

serve to decrease or eliminate the risk of out of home placement.

● If a crisis is unavoidable, and the behaviors warrant a presentation to the CRC or

formalized crisis response, a crisis consultation between the FOCUS CM and the CBH

CCM must be completed within 3 days of crisis presentation or crisis incident. If a

Physician Advisor consult is needed to support, the CCM will schedule a consultation to

include the case manager, CCM, and Physician Advisor.

● Provide 24-hour crisis response. All FOCUS CM utilize the Ring Central Communication

system to receive calls from members/ families in the event of a MH/ BH emergency.

● Participate in bi-weekly checks-in with the FOCUS CM and the CBH BHL to provide a

status update and any significant clinical concerns or risks will be flagged for CCM

follow up care coordination.

● Participate in bi-weekly care coordination meetings with CBH and review clinical and

system challenges that impact members during their treatment episode. Team members

will conceptualize the context of challenges experienced and identify solutions that will

decrease or eliminate identified issues that impact treatment/care or community tenure.

● Electronic chart maintenance to ensure compliance standards requiring psychiatric,

medical, and school records compilation

● Update and maintain all case management records including telephonic progress updates

and treatment consultations with CBH approved psychiatrist/psychologist.

● Participate in interagency team meetings at home, school, and all collaborating agencies.

● Make home, school, hospital and community-based visits at a minimum of 2 times per

month or as needed.

● Other responsibilities as designated by the supervisor.


QUALIFICATIONS AND EDUCATIONS REQUIREMENTS

Bachelor’s Degree in sociology, social work, psychology, or other social science and two (2)

years experience in Intensive Case Management. Master’s Degree preferred. Working knowledge

of Crisis Intervention, the Philadelphia Behavioral Health and Juvenile System, Community

Resources (i.e.Department of Public Welfare, Educational Law, Office of Social Security and
