Referral Coordinator

Monday through Friday 8:30 to 4:30pm schedule.

Our Health Center offers a range of primary and specialty health care for adults and children. We provide specialized clinics, Eye Care, Dental, Pediatrics, and OB/GYN.

Summary/Objective:
The Referrals Coordinator is responsible for completing all incoming and outgoing referrals for their assigned clinic in a timely manner, and in accordance with established policy and protocol. They are particularly key in the effort to deliver coordinated patient-centered care. The Coordinator reviews and processes all diagnostic, specialty, and miscellaneous referral requests, obtains authorization for referrals, and schedules appointments for patients as required. Works closely with providers, patients and payers in a team-based manner to achieve a positive impact on the outcome, cost, and quality of patient-centered care rendered. They build relationships and communicates effectively with providers, patients, and payers as appropriate.

Essential Functions

1. Reviews and processes all diagnostic, specialty, and miscellaneous referral requests from providers using ECW.

2. Demonstrates mature judgment and complete confidentiality regarding patient’s medical information and adheres to HIPPA compliance. Refrains from discussing patient information in public areas, etc.

3. Returns patient’s Medical Record to Medical Records Department or a locked cabinet when no longer in use (when accessing older records not found in ECW).

4. Obtains authorization for referrals and schedules all appointments accurately and in accordance with established department-specific guidelines.

5. Reviews and verifies eligibility of benefits.

6. Makes reminder calls for all appointments scheduled and records result in the current database in accordance with established policy and procedure. Schedules follow-up patient appointments in the current system.

7. Generates reports as required for department services (pending referrals, completed referrals) and maintains all referrals in an appropriate database.

8. Photocopies, faxes and scans all referrals materials as required. Electronically inputs referrals data as required.

9. Maintains a clean, organized work environment.

10. Attends staff meetings as required, and other duties as assigned.

1. Self-directed, self-confident, and able to work with minimal direction.

2. Able to effectively communicate with all levels within clinical departments. (English and Spanish fluency)

3. Detailed oriented and organized, with excellent follow through skills.

4. Excellent problem-solving skills, and the ability to be a team player.

Work Environment
This job operates in an office environment.

Position Type/Expected Hours of Work
This position will operate on a Monday through Friday, 8:30am to 4:40pm.

Compensation: $21-$23 per hour

Required Education and Experience

1. High School Diploma or G.E.D required.

2. At least 2-3 years demonstrated experience in an ambulatory environment required.
