Overview

The Vermont Department of Health has a dynamic opportunity with the Middlebury Office of Local Health for an enthusiastic and experienced public health professional who wants to make a difference in the health of communities in Addison County Vermont. We are seeking an organized and creative individual who enjoys collaborating with healthcare and community partners and is passionate about removing barriers to achieve optimal health for all Vermonters. 


The Chronic Disease Public Health Specialist supports various health department initiatives to improve health equity and health outcomes in and around Addison County. The focus of this position will be on providing chronic disease prevention education as well as local systems and policy-level work with community partners to improve the health of populations experiencing the greatest disparities: Black, Indigenous, People of Color, LGBTQ+, people with disabilities, refugees, immigrants, migrant workers, low-income populations, and others. Examples of focus areas include improving chronic disease health outcomes related to food security and nutrition, physical activity, tobacco use, oral health, healthy aging, healthy community design, worksite wellness, and encouraging preventative screenings for cancer. 

 

This position also works with a multi-disciplinary public health team on emergency preparedness and response activities. Duties are performed under the supervision of the Public Health Services District Director. This position is part of a cohesive, motivated, and hard-working multi-disciplinary public health team that enjoys community and person-centered approaches to public health, ongoing learning, collaboration, and creating a productive, balanced, respectful, and joyful work environment.
