A Brief Overview
The Cardiovascular Health Destination Service Line Liaison, under general supervision and guidance of clinic manager and the program care team (registered nurse coordinators, advanced practice providers and attending physicians) performs new patient coordination, surgery scheduling, master scheduling, authorization coordination, specialized data collection and coordination of their designated program in the clinic. The scope of responsibilities includes patient flow management, scheduling of complex appointments, procedures and surgeries requiring strong knowledge of procedures, department protocols, and timing, administrative and resource coordination for their assigned program. Performs coordination of transitions of care scheduling for Cardiovascular Health Destination specialty programs in Cardiovascular Medicine, Cardiothoracic Surgery and Vascular Surgery. The CVH Liaison also collaborates with management to evaluate, recommend, and conduct performance improvements and workflow efficiencies within their scope of practice. This position serves as a standard across the Stanford Health Care enterprise for the Cardiovascular Health Destination Service Line.

What you will do
- Specialized functions include complex appointment, procedures and surgery scheduling requiring strong knowledge of procedures and department protocols.
- Interprofessional collaboration with physicians, APPs, RNs, and other department staff in their designated program.
- Complex care coordination: procedure/surgical add-ons, admissions, multi-disciplinary procedure/surgical clearances, inpatient transfers, blood bank.
- Patient information management between providers, inpatient/outpatient care teams, and ancillary services.
- Functions independently to complete activities related to the registration process. Responsible for completeness, accuracy, and timeliness of data.
- Coordinates and facilitates authorization process.
- Serves as a resource to other staff in registration, benefits, and authorization process.
- Reviews and analyzes all registration edit reports and ensures that corrections to the database are made in a timely manner
- Maintains master appointment schedule.
- Manages new patient coordination: intake, new patient appointment scheduling, scheduling of appropriate testing, and record retrieval including diagnostic test results.
- Coordinates day to day support operations and services for the program.
- Ensures the timely completion of work assignments.
- Responsible for workplace safety and infection prevention within the scope of the role.
Education Qualifications
- High School Diploma or GED equivalent Preferred: Associates degree in health-related field from an accredited college or university OR Bachelor’s degree
Experience Qualifications
- 1-3 years of directly related experience
