About the Role:
We are seeking a dedicated and collaborative Nurse Practitioner or Physician Assistant (Advanced Practice Practitioner, APP) specialized in Endocrinology and Obesity Medicine to join our innovative, multidisciplinary chronic disease management team. This new role will involve using a lifestyle and obesity medicine approach to prevent and treat cardiometabolic conditions including comprehensive diabetes management.
The majority of your patient population will primarily focus on obesity/diabetes management across the full spectrum of disease, including Type 1, Type 2, post-transplant, and other complex secondary forms of diabetes (ex steroid-induced, 2nd to pancreatic disease, Cystic fibrosis related, MODY, etc). Endocrine APPs will help enhance the consistency, safety, and effectiveness of patient care across the newly formed endocrine team, specifically integrating the use of Continuous Glucose Monitors (CGMs), complex insulin regimens, and insulin pump therapy.
This is a full-time position based in a remote, telemedicine clinical setting. The APP will work closely with a supportive care team and have protected time for supervision, teaching, and administrative duties.
What You Will Do:
- Provide direct patient care in a telemedicine clinical setting, focusing on evidence-based treatment for obesity and comprehensive diabetes management, including titration of insulin and advanced use of diabetes technology (CGMs/pumps).
- Collaborate with Endocrinologist colleagues and Registered Dietitians specializing in diabetes treatment to co-manage a shared patient panel.
- Review charts and conduct return visits for patients to ensure quality, continuity of care, and appropriate management of CKD/ESRD and CAD/CV conditions.
- Provide advanced treatment planning and medical management support for complex endocrinology cases, escalated concerns, and patients requiring specialized management protocols.
- Prescribe non-controlled medications, including those for obesity and diabetes, for patients under both direct care and in coordination with Endocrinology collaborators.
- Conduct weekly 30 minute collaborative meetings and 30 min asynchronous communication time with collaborating physicians to review cases, address clinical challenges, and provide ongoing support.
- Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team, with a focus on guidelines for managing complex diabetic populations.
- Assist in the professional development of colleagues by fostering clinical growth, offering feedback, and supporting continuing education in both endocrinology and obesity medicine.
- Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.
- Attend relevant departmental meetings and contribute to quality improvement efforts within the diabetes and obesity medicine program.
About You:
- Required: at least 2 years practicing advanced diabetes management, including the use of CGMs, insulin pumps, and complex insulin regimens, able to sign a collaboration agreement with an Endocrinologist
- Preferred: OMA certification (CAEOM) or demonstrated clinical experien
