What You'll Do:

  • Provide physical therapy and patient care in various settings, including our outpatient clinic, acute hospital inpatient, and post-acute swing bed unit, with opportunity to expand into outreach services including home health and school settings.
  • Work with patients and families to examine, evaluate, diagnose, prognose, and treat to ensure optimal outcomes.
  • Educate patients, caregivers, and family members as needed.
  • Collaborate with the referring providers and multidisciplinary teams in providing optimal patient care through appropriate and timely methods of communication.  

What's Available:

  • Fulltime, 80 hours bi-weekly (1.0 FTE).
  • Days; Mondays - Fridays with weekend on-call rotation.

What You'll Need:  

  • Bachelor's degree, or higher, in Physical Therapy from an American Physical Therapy Association (APTA) approved curriculum in Physical Therapy
  • State of Wisconsin Physical Therapist License 
  • Current Infant-Adult BLS for Health Care Providers from American Heart Association or American Red Cross.

Job Description:

The Physical Therapist in their clinical role examines, evaluates and assesses individuals of all ages who have medical problems or other health related conditions that limit their ability to move or function with activities of daily living. The Physical Therapist develops a plan of care and provides treatment techniques and interventions to improve mobility, reduce pain, restore function and prevent disability. The Physical Therapist provides physical therapy services and elements of patient/client care management leading to optimal outcomes according to the relevant state Physical Therapy Practice Standards. The Physical Therapist works collaboratively with a multidisciplinary team in a team in the areas of education and consultation regarding patient needs in accordance with the vision and mission of Gundersen Health System.

Major Responsibilities:
